ASC - Social Worker (Older Adult Community Mental Health Team)

Who You Are

You are a qualified social worker who is eager to make a difference in the lives of the elderly. You are comfortable operating in a multi-disciplinary older adult mental health team, and have experience with various facets of social work, including assessment, planning, and review. Your passion for helping others allows you to provide the necessary support and care to this vulnerable population.

What the job involves

  • Carrying a caseload to provide monitoring and support
  • Undertaking initial assessments of need
  • Formulating care plans, risk assessments, and assessing capacity on specific decisions
  • Offering and undertaking carer assessments
  • Planning discharge from hospital, bringing cases for funded service requests as required
  • Undertaking reviews of packages of care, placements and other funded services
  • Acting as an enquiry officer for adult safeguarding
  • Acting as one of the duty workers as part of a rota

Skills

  • Trained and certified in social work
  • Experience in a multi-disciplinary team
  • Adept at assessment and care plan formulation
  • Able to conduct risk assessments
  • Experienced in hospital discharge planning
  • Proficient in reviewing care packages and placements
  • Comfortable acting as a duty worker
  • Knowledgeable in adult safeguarding
