Who You Are
You are an integral part of the Central Monmouthshire Integrated Team, dedicated to providing health and social care services that enable people to maintain their independence and avoid unnecessary hospital admissions or residential placements. You will work closely with your team to support good hospital discharges and provide excellent community-based care.
What the Job Involves
The role involves collaborating with team members and adhering to organizational policies to ensure the effective delivery of services that allow individuals to stay in their homes safely. You will report to Kelly Davies, the Team Manager, and Sue Kempson, the Senior Social Work Practitioner. Your work will include complying with the organization's no smoking policy and making necessary adjustments to your duties as required to adapt to the evolving needs of the organization.
